Riedel's Thyroiditis Diagnosis

Clinical differentiation from anaplastic carcinoma is very important: both may present with a hard, rapidly growing neck mass. Tissue sampling in Riedel's thyroiditis may show a specific fibrous tissue pattern and IgG4+ plasma cell infiltration. Serum IgG4 level is usually elevated. CT/MRI evaluates the extent of fibrous invasion and relationship with surrounding structures.

Systemic IgG4-RD investigation is planned according to clinical context and may also involve the pituitary gland, retroperitoneum, mediastinum, biliary ducts and lacrimal glands. Hypothyroidism is often the dominant clinical picture; TSH is high and fT4 is low. Hypoparathyroidism may result from fibrous invasion affecting the parathyroid glands.

Treatment

Tamoxifen and glucocorticoids are among reported medical options in Riedel's thyroiditis; dose and duration are defined by a specialist together with disease activity, accompanying IgG4-RD findings and side-effect risk.

Surgical isthmus resection may be assessed for airway safety; total thyroidectomy can be risky because of fibrous adhesions, and forced resection may injure the RLN. Levothyroxine replacement for hypothyroidism and biological therapies in the IgG4-RD context are discussed through multidisciplinary assessment.

Prognosis

Riedel's thyroiditis may follow a chronic course; complete recovery is rare. Long-term stable disease has been reported with tamoxifen. Life-threatening complications include airway obstruction and hypocalcemia due to hypoparathyroidism.

In cases associated with systemic IgG4-RD, prognosis varies according to the accompanying organ involvement. Long-term permanent hypothyroidism and hypoparathyroidism are important sequelae; follow-up interval is shaped by endocrinology and relevant specialties together.

When to Seek Care

A stone-hard, rapidly growing neck mass together with swallowing or breathing difficulty calls for priority assessment; anaplastic carcinoma and Riedel's thyroiditis are considered together in the differential. Differentiation is made through tissue diagnosis and imaging context.

For patients already being followed in the context of other IgG4-RD (autoimmune pancreatitis, sclerosing cholangitis, etc.), thyroid gland involvement is reviewed according to clinical findings.
