Components of the Test Battery
VNG/ENG caloric testing stimulates the horizontal semicircular canal with 44°C warm and 30°C cool water or air; canal paresis percentage and directional preponderance are calculated. Video head impulse test (vHIT) evaluates high-frequency VOR gain and corrective saccades with high-acceleration head movements; it can be applied bedside.
Cervical VEMP (cVEMP) records the inhibitory potential of the saccule to acoustic stimulation from the sternocleidomastoid muscle; it demonstrates inferior vestibular nerve and saccule integrity. Ocular VEMP (oVEMP) evaluates the utricle and superior vestibular nerve; amplitude markedly increases in SSCD. Computerised dynamic posturography measures balance strategies by modifying surface and visual conditions.
Clinical Value of the Test Battery
The vestibular test battery is valuable not only for diagnosis but also for monitoring treatment response: normalisation of the caloric test over time in vestibular neuritis confirms central compensation. In Menière's disease, canal paresis changes with attack frequency and treatment can be followed. Residual function can be assessed after intratympanic gentamicin ablation.
In rehabilitation programme planning, posturography results determine which sensory system (visual, proprioceptive, vestibular) is dominant and the exercise target. Determination of functional reserve before surgery (labyrinthectomy, endolymphatic sac surgery) contributes to the surgical decision.
Interpreting the Test Battery
A single test result is insufficient; a consistent combination of findings increases diagnostic reliability. For example, normal vHIT but abnormal caloric test indicates low-frequency selective VOR impairment and carries a different prognostic meaning. Completing the test battery and integrating findings with the clinical picture requires specialist interpretation.
Age, medication use (particularly sedatives and vestibular suppressants) and level of cooperation affect test results. Suppressants may suppress caloric response if not discontinued at least 48 hours beforehand.
When Should Vestibular Testing Be Done?
Every patient with dizziness, imbalance or risk of falling benefits from vestibular evaluation. Comprehensive test battery is recommended in vestibular neuritis, Menière's disease, non-BPPV recurrent vertigo or suspected acoustic neuroma.
Unexplained fall history in elderly patients requires vestibular function assessment. Baseline evaluation before intratympanic treatment or vestibular surgery and post-treatment follow-up monitoring is mandatory.
