Diagnosing Ear Canal Exostosis

Otoscopy shows broad-based smooth-surfaced bony protrusions close to the bony-cartilaginous junction. Distinction from osteoma is important: exostosis is multiple, bilateral and broad-based while osteoma is single, pedunculated and made of lamellar bone. High-resolution CT examines the lesions and determines how much of the canal they occlude.

Hearing may vary depending on the degree of canal obstruction; pure-tone audiometry and tympanometry are part of the assessment protocol. History of recurrent OE and occupational/hobby history related to water sports are guiding clues for exostosis.

Treatment: Canalplasty

Canalplasty is performed under general anaesthesia; exostoses are removed with specialised drill tips via endoscopic or microscopic approach. A skin flap is elevated via postauricular or endaural incision, bone is drilled and skin is returned to position. Success rates exceed 90% in experienced hands; major complications (facial nerve, tympanic membrane damage) are rare.

Post-operative healing takes 4-6 weeks; water contact should be restricted during this period. Return to water sports requires both use of protective equipment and regular ENT follow-up because exostosis can recur. Medical treatment (hearing aid fitting, OE prophylaxis) can be evaluated before surgery.

Prognosis

Symptom control and reduction of OE attacks after surgery are successful in the large majority. Risk of recurrence occurs when cold water exposure continues; protective equipment is therefore critically important. Audiological improvement — resolution of conductive loss due to cerumen impaction — is achieved in many patients.

The disadvantage of delaying the surgical decision is increased cerumen accumulation and rising frequency of recurrent OE in canal-encircling narrowing. Annual follow-up is appropriate for asymptomatic lesions.

When to Seek Medical Care

Individuals who engage in water sports should have an ear check-up annually. Recurrent ear infections, cerumen accumulation or hearing reduction are sufficient reasons for ENT evaluation. Otoscopy and audiometry should be performed when exostosis is suspected.

Sudden hearing loss, high fever or severe pain may indicate a more serious picture than expected; urgent evaluation is needed in this situation. ENT specialist referral to discuss surgical indication should be planned in patients with diagnosed exostosis.
