Diagnosing Vestibular Neuritis
Diagnosis is clinical; the triad of sudden-onset severe rotational vertigo, nausea-vomiting and normal hearing constitutes the typical picture. Video head impulse test (vHIT) reveals reduced high-frequency VOR gain in the affected canal. Caloric testing confirms canal paresis in the affected ear.
Central vertigo originating from the brainstem or cerebellum must be excluded; the HINTS protocol (head impulse, nystagmus type, test of skew) is useful in the emergency setting. MRI is not required to exclude a central lesion but should be obtained when atypical findings, new hearing loss or neurological signs are present.
Treatment of Vestibular Neuritis
Early oral or IV steroid treatment may support balance recovery in selected patients. Vestibular suppressants such as dimenhydrinate are given briefly for acute nausea-vomiting control; prolonged use can delay compensation. Active exercise and mobilisation are planned as symptoms allow.
Vestibular rehabilitation therapy includes adaptation, substitution and balance exercises; structured VRT programmes have been shown superior to placebo in randomised trials. Home exercise programmes are as effective as clinic-based sessions.
Prognosis
