Functional Rhinoplasty in Istanbul: Breathing Better and Looking Natural

Functional rhinoplasty is a surgical approach that simultaneously improves both the internal structure and external appearance of the nose. Unlike purely cosmetic rhinoplasty, the primary goal is to restore breathing function, with aesthetic improvement as a secondary gain. It is therefore recognised as a specialty spanning both ENT surgery and plastic surgery disciplines.

The most commonly treated conditions include: deviated septum (curvature of the nasal septum), inferior turbinate hypertrophy (abnormal enlargement of the nasal turbinates), and nasal valve insufficiency (narrowing of the nasal inlet). These three conditions frequently co-exist and must be addressed together to achieve adequate breathing.

Functional rhinoplasty is also performed in a revision context to correct adverse effects that a previous rhinoplasty has had on breathing. This specific situation is called 'revision functional rhinoplasty' and is technically more demanding.

Istanbul is one of the highest-volume surgical centres globally, performing tens of thousands of rhinoplasty procedures per year. This volume allows surgeons to accumulate broad case experience rapidly. Turkey's medical faculties meet European standards for ENT specialty training, and many surgeons complete fellowship programmes in Europe or North America.

In closed rhinoplasty all incisions remain inside the nose; no visible scar is left on the columella. In open rhinoplasty a small incision is made across the columella, nasal tissues are elevated and the surgeon accesses all structures under direct vision. In functional cases, especially complex nasal valve problems or severe septal deviations, the open approach is preferred because more precise technical corrections can be made.

Septoplasty (septal correction) and inferior turbinate reduction (inferior turbinoplasty or laser turbinate reduction) are the core components of functional rhinoplasty. When valve reconstruction is required, cartilage grafts are used; the most common graft sources are intranasal cartilage and auricular (ear) cartilage.

In the first 48 hours after surgery, nasal packing or splints remain in place. During week one, swelling and bruising are expected; the patient can return to social life within 10-14 days. The nasal cast is typically removed at 7-10 days. The final shape has not been reached in the first three months; the majority of swelling subsides by month six. The full result is assessed at 12 months.

Breathing improvement is felt sooner than the aesthetic improvement: after packing removal, most patients report a noticeable increase in breathing quality. Mouth breathing may be necessary until nasal splints are removed; this is temporary.
