Rhinoplasty vs Septoplasty: Understanding the Difference

Rhinoplasty is aesthetic or functional surgery performed to change the external appearance of the nose (hump removal, tip refinement, shape change). Septoplasty is an ENT procedure that opens the airway by correcting the curvature of the septum (cartilage-bone partition) inside the nose. Both can be combined in the same session: this combination is called 'septorhinoplasty' and improves both appearance and function.

Rhinoplasty: appearance-focused surgery
Rhinoplasty is surgery that changes the size, shape and proportions of the nose. The most common changes are: dorsal hump correction, nasal tip refinement or elevation, width reduction, changing alar size and correcting asymmetry. In primary rhinoplasty all of these are performed with intact natural tissue while in revision cases scar tissue creates difficulty.
Rhinoplasty can be performed with open (external incision) or closed (entirely internal incision) technique. In closed technique no scar is visible; in open technique there is a very small columella scar. The indications and advantages of both techniques differ; the choice is based on the surgeon's assessment.
Septoplasty: breathing-focused surgery
Septoplasty is a procedure performed entirely inside the nose, leaving no visible external scar. The surgeon raises a mucoperichondrial flap and reshapes the deviated cartilage and bone. As a result the airway widens; the external appearance of the nose does not change.
Septoplasty is usually combined with inferior turbinate reduction (inferior turbinoplasty) because turbinate hypertrophy accompanies septal deviation. This combination optimises breathing improvement. Surgery takes 45-90 minutes; it is a day-case or single-night procedure.
Septorhinoplasty: the best of both worlds
Septorhinoplasty is ideal for patients with both breathing problems and aesthetic concerns at the same time. Both the septum and external structure are corrected in a single anaesthetic, single recovery and single operation. This combined approach provides a clear advantage for the patient compared to two separate operations.
In septorhinoplasty the surgeon works in collaboration: septal cartilage can be used as a graft source for aesthetic reconstruction. For example spreader grafts or tip grafts used for nasal tip support are prepared from cartilage obtained from the septum. Therefore aesthetic and functional dimensions require intertwined planning.
Insurance and cost difference
When septoplasty is based on a medical indication (airway obstruction, contribution to obstructive sleep apnoea) it can be covered by state and private health insurance. Purely aesthetic rhinoplasty is outside insurance coverage unless there is a medical justification.
In septorhinoplasty the costs of two components are evaluated separately: the functional component (septoplasty, turbinate reduction) can be covered by insurance while the aesthetic component (external shape change) is invoiced as the portion the patient pays themselves. This distinction must be made carefully in the clinical accounting process.
