Interpreting Tympanometry Curves
A Type A curve with peak around 0 daPa and normal compliance indicates normal middle ear function. A Type Ad curve with high compliance suggests ossicular chain discontinuity or hypercompliant membrane. A Type As curve with low compliance suggests otosclerosis or tympanosclerosis of the membrane.
A Type B curve is interpreted according to volume: small volume (<0.8 mL) indicates effusion, large volume (>2 mL) indicates perforation or ventilation tube, and intermediate volume may indicate the probe being wedged in the ear canal. Type C with peak beyond -200 daPa reflects Eustachian tube obstruction.
Management Based on Tympanometry Result
If the Type B effusion curve persists for 3 months or longer, ventilation tube indication is evaluated. A Type C negative pressure curve is monitored with conservative approaches such as Eustachian tube exercises, topical corticosteroid or nasal balloon inflation. If resistance is not broken, balloon Eustachian tuboplasty may be considered.
A Type Ad curve requires detailed ossicular chain evaluation; CT and surgical exploration can be planned. Type As is managed with stapedectomy/stapedotomy when otosclerosis is suspected. In large-volume Type B curve where perforation is detected, the myringoplasty decision is made after a 3-month waiting period.
Clinical Reliability
Tympanometry is a reliable, inexpensive and painless test; it is an integral part of ENT examination. In newborns and infants, a 1000 Hz probe tone gives more reliable results than standard 226 Hz because the outer ear canal wall compliance is high in infants. Test results should always be interpreted together with clinical assessment.
Cerumen in the external auditory canal, air leaks or cooperation problems can lead to measurement errors. Tympanometry does not diagnose on its own; integrated assessment with audiometry, otoscopy and symptoms improves clinical decision quality.
When Should Tympanometry Be Done?
In children with recurrent ear infections, suspected hearing loss or speech delay, tympanometry is the first evaluation tool. In adults it should be performed in every patient with ear fullness, conductive hearing loss or Eustachian tube complaints.
It is a standard part of pre- and post-operative audiological assessment. If ear pain or hearing reduction occurs after barotrauma (flight, diving), tympanometry is requested together with urgent ENT evaluation.
