Diagnosis
IV contrast CT (1–1.5 mm orbital cuts) is mandatory. Stages III–IV: emergency endoscopic drainage plus FESS. Stage V: ICU, LMWH anticoagulation (controversial), neurosurgery plus ophthalmology consultation. The risk of visual loss necessitates drainage within 24 hours.
Outcomes and Follow-up
Stage I–II complete recovery with antibiotics >95%; progression to orbital abscess (III–IV) 5%. Visual loss risk without timely surgery in Stage III–IV is 10–20%. Stage V mortality is 15–30%. Visual acuity and EOM recovery depends on duration and severity of pressure on the optic nerve. Recurrence after subperiosteal abscess drainage is <5%.
When to Seek Care
A child with periorbital swelling after URI/sinusitis must present to the emergency department the same day. Proptosis, diplopia, visual change or restricted eye movement constitutes a surgical emergency requiring CT within 2 hours. When RAPD is detected on flashlight test, an emergency ENT call must be made.
