Differences, limits, and selection criteria for home respiratory testing and laboratory polysomnography in obstructive sleep apnoea.

What does this condition mean?
Polysomnography records sleep stages, brain waves, eye and muscle activity along with breathing, oxygen, heart rhythm, and body position. A home sleep apnoea test or respiratory polygraphy uses fewer channels and mainly measures breathing and oxygen. The two methods therefore do not provide the same amount of information.
- Sleep apnoea is not diagnosed by snoring, a questionnaire, or a smartwatch alone. A clinician-ordered and interpreted home test may suit selected uncomplicated adults with moderate-to-high likelihood; a negative, inadequate, or discordant result and complex illness require laboratory polysomnography.

A home sleep apnoea test or respiratory polygraphy uses fewer channels and mainly measures breathing and oxygen. The two methods therefore do not provide the same amount of information.
Which clues help distinguish the causes?
Witnessed pauses, loud snoring, waking with choking, daytime sleepiness, resistant hypertension, and obesity increase pre-test likelihood. Symptom intensity does not reliably grade apnoea. Because a home test may use recording time rather than true sleep time, it can underestimate events in some people.
How is it assessed in ENT practice?
The test is chosen after a detailed sleep and medical history. Polysomnography is more suitable with significant heart or lung disease, neuromuscular weakness, suspected hypoventilation, chronic opioid use, previous stroke, severe insomnia, or suspected additional sleep disorder. Children and special groups do not follow the routine adult home-test pathway.
Why is treatment individualized?
The result is interpreted with symptoms, oxygen burden, sleep position, anatomy, and coexisting disease rather than as one number. Positive airway pressure, oral appliance, weight management, positional therapy, or surgery are selected within that whole picture. The same AHI does not mandate the same treatment for everyone.
When should help not be delayed?
Stop driving and seek rapid assessment if you doze at the wheel or have near misses. New chest pain, blue discoloration, severe breathlessness, altered consciousness, or a new neurological deficit is an emergency and should not wait for a sleep-test appointment.
